The company has considered its members as a family member and therefore always considered their sorrow and pleasure as its own and to help them is a noble duty of the company. With this spirit the company has launched a new scheme which shall cover all members who are enrolled w.e.f. 1ST september, 2008.according to this scheme if any member whose age is in between 18 years to 50 years dies normal death the company shall pay a sum of rs. 51,000/- ie other than the accidental death the company shall pay a sum of rs. 51,000/-(plan1) and the company shall pay a sum of rs. 1,00,000/-(plan2), to his nominee as consolation monetary help as per following terms and conditions:-

This benefit is admissible to every member for a period of 4 years from the date of joining which shall be effective after one year from the date of his joining.
If any member has achieved the status of stage s-1 as per performance chart, this benefit shall be extended up to the age of 50 years of such member.
Such death should occur after one year form the date of membership and should not be as a result of accident. Here accident means all such incidents as mentioned in accidental policy
In case of accidental death only the amount under accidental policy is payable.
In case whose age is in between 18 years to 50 years dies normal death the company shall pay a sum of rs. 51,000/- ie other than the accidental death the company shall pay a sum of rs. 51,000/-(plan1) and the company shall pay a sum of rs. 1,00,000/-(plan2), as per following terms:

The nominee shall give written intimation to the company within seven days from such death.
A claim form of the company duly filled in is to be submitted to the company along with following documents-

Certificate of date of birth
Death certificate
Cause of death with authentic proof
Passport size photo of deceased and nominee

On receipt of the claim the company shall get the investigation/survey done and on receipt of such survey report the claim settlement shall be done.
In normal course, the claim shall be settled with in a period of three months from the date of receipt of such claim.
The claimant shall submit a pre-receipted discharge voucher duly signed on revenue stamp and shall also mention their bank account number.

In case of normal death at the age of more than 50 years and in case of accidental death at the age of more than 70 years, no amount is payable by the company.

Ans.: For five family members, (you, your wife, three non married dependent child blow 18 years), as per regulations under case of hospital admit hospital expenses will be provided by ccm pariwar on annual basis.
